Monitoring helps organise screening information recorded at different times, making it easier for authorised personnel to review available history and support appropriate follow-up.
SCREEN. RECORD. REFER.RenalTrack is being developed to support the recording and longitudinal monitoring of selected urine-test screening results. Where relevant history and review features are implemented, authorised personnel can use available records to understand what was documented at different encounters.
Monitoring does not automatically explain why a result changed, establish a diagnosis or predict a person's health outcome. Findings must be interpreted within the limits of the test and the person's circumstances.
The exact actions available depend on the deployed RenalTrack version and the approved screening workflow.
Capture the screening information using the approved procedure and ensure that the entry is associated with the correct encounter.
Keep screening records organised so that available earlier entries can be retrieved and reviewed by authorised users where supported.
Consider recorded observations alongside their dates, testing method, relevant circumstances and any documented limitations.
Where findings need attention, use the approved pathway for professional assessment, further testing or referral.
A well-organised history can help authorised personnel understand what was recorded previously and identify information that may need review. Its usefulness depends on the quality and completeness of the records.
Keep available screening encounters organised rather than relying entirely on disconnected notes or recollection.
Help authorised personnel refer to earlier entries when considering a new screening encounter.
Support attention to incomplete, unclear or inconsistent documentation through appropriate review procedures.
Provide relevant documented history to support discussions between participants and qualified healthcare professionals.
Differences between screening observations may reflect a real change, variation in testing conditions, specimen handling, the product used, data-entry errors or other circumstances. A record history alone cannot establish the cause.
A qualified healthcare professional should determine whether further assessment is appropriate. RenalTrack must not be used as a substitute for clinical judgement, confirmatory investigations or necessary care.
Recorded observations are only as useful as the procedure behind them. The exact urine-test strip brand and model, manufacturer reference chart, instructions and reading times must be confirmed for the intended screening workflow.
RenalTrack's result mapping must not rely on guessed colour ranges, unverified thresholds or assumptions borrowed from another product. Until the applicable product documentation has been verified, specific result interpretations should not be treated as ready for operational use.
